To apply for The University Chrisitian Fellowship School of Supernatural Ministry, kindly fill out the the  pastor reccomendation form and the application form.

Please open the following pastoral recommendation form, print them out and give them to your pastor (or someone who has been your spiritual mentor) to be filled out.

Note: If you prefer, you can do your personal application and send it by email to us.
In regards to your personal application, you can

• Print out a hard copy, fill it and mail it back.
• If you have a Windows version of 2007 or anything newer, you can fill out the form in Word itself. Download the file 'SSM Application.docx', save it to a place in your computer where you will be able to access it later, fill it out and email it back to us at j_lortie@bellsouth.net.
• If your version of Windows is older than the 2007 version, save the document to your computer (in a place where you can find it after you save it) and open it in Microsoft Word. Then, delete the lines, WHERE NECESSARY(where you would normally write your answers) and type in your answers, leaving spaces to see clearly the questions and your answers on the application. When you are done you can email your application to us at j_lortie@bellsouth.net.
